Scheduling Appointments

We know that your time is valuable, which is why we make extra efforts to see our patients at their scheduled times. Unfortunately, our office does occasionally fall behind schedule, so we ask for your understanding. We also ask that you give us as much advance notice as possible if you are running late for your appointment or you need to cancel. This advance notice will help us schedule the time for someone else’s appointment.

What to Bring to Your Appointment

If you are a first-time patient, please plan on arriving early in order to complete a patient registration form, and please bring the following items to the appointment:

  • Insurance card(s)
  • Referral forms (if required)
  • List of all prescription and over-the-counter medications that you are currently taking
  • Any required co-payment
  • Pertinent medical and surgical history information
  • X-ray films
  • Foot orthotics

We encourage all patients to confirm their coverage with us prior to making an appointment. All co-payments are due at the time of your appointment. If your insurance company does not pay your full balance or denies the claim, then your unpaid balance is due within 30 days of your appointment.

Please call us with any questions regarding which insurance plans we accept or any other billing concerns you may have. If you have any insurance coverage and benefit questions, you should direct them to your insurance company.
